Are any of your children married? If Yes which of them and what are their spouses full names? *

Please state any specific requirements for the guardianship of your child/children? *

If divorced, Are there any peculiar circumstance regarding your former marriage which should be put into consideration? *

Do you have an antenuptial (Pre-Marital) Contract?. If you do please provide us with a copy. *

Do you make this will having a sound disposing mind? *

Who do you intend to appoint as your Executors and Executrixes? (There must be at least 2 executors) Note that where you appoint a law firm as an executor, they are entitled to charge and be paid usual fees for work done, services rendered and time spent in the administration of your estate. Please include names of substitutes *

What are your instructions regarding any bequests to minor children? *

If you choose to use a trust, please state the full names of the Trustees. Note that trustees could be the same persons as the executors. (They must be at least 2 trustees) *
